Required permissions
You must have the required permissions to view return request notices.
Remove personal information
Before sharing or storing the document, remove any personal information, such as addresses or other identifying details.
On your My Portal dashboard, select the Menu icon in the top-left corner to open the navigation panel.
Move your cursor over Devices & Supplies and select Return Requests.
Find the return request notice you want to open and select the Reports icon.
Select Equipment Return Request Notice.
The notice downloads automatically.
Save the file to your computer.
Open the PDF to view the notice.
Follow the instructions in the document to arrange the return of the device to Clario.
Equipment should be returned in a timely manner according to the return instructions.
