Managing Modality Assessments

The Modalities (e.g., MR) are listed in the left column. A gray card indicates the selected modality. All related information, including the Modality Assessment, appears on the right.

1. Click Start Questionnaire.

By default, the Sites page shows the Modalities tab. If you're on the Site Users tab, click the Modalities tab to access the Modality Assessment.

The online form may include various question and answer formats:

• Single Choice: Click the radio button to select your answer.

• Multiple Choice: Click multiple radio buttons to select all applicable options.

• Free-Text: Type your response in the provided field.

• Numerical: Click the radio button to select your numerical answer.

• Date/Time: Choose dates and/or times when prompted.

Remember to answer follow-up questions as needed.

Submit Responses

1. Click Submit to finalize your responses. A notification will confirm the form was submitted.

2. A "Completed" status will show next to the Open Questionnaire button.

Save as Draft

1. Click Save as Draft to save and exit. You can edit later. A notification will confirm the form is saved.

Dismiss Form

1. Click x to dismiss the form.

Unsaved responses will be lost. Saved responses remain unaffected.
