    This guidance applies to clinical sites outside of the U.S. returning items to Clario’s U.S. facility in Bristol, PA.


    Before Starting

    Determine if this guidance applies to your Clario returns.

    I am returning an eCOA device.

    I am sending my return to Clario's U.S. facility in Bristol, PA.

    My clinical site located outside the U.S.

    The device has not been used for more than three ('3') years.*

    *If the device has been in use for over 3 years since its original U.S. Export Date, do not complete or include the declaration in the return.


    Finding the FSD

    1. Go to the Clario-specific UPS online portal.

    2. Click Continue As Guest.

    3. Select the link Return Inventory Form + Shipper Declaration.

    4. Open the Word document Foreign_Shippers_Declaration_Returned_Goods (1).

    The UPS online portal is still branded for Clario’s legacy business, ERT. You are in the right place.


    Completing the FSD

    1. Copy the FSD provided by Clario.

    2. Paste it into your company’s letterhead.

    3. Enter the Original U.S. Export Date.

    4. Enter your name and title as the authorized representative.

    5. Sign your name in the signature field.

    6. Enter the date in the date field.

    7. Securely attach the FSD to the outside of the box a clear pouch or transparent tape, making sure it's easily readable by U.S. Customs officials.

    You may need to reformat the FSD form on your letterhead, as the original formatting won't apply.


    Finding the “Original U.S. Export Date” Using Clario Reports

    1. Enter your user credentials at: Clario Portal login.

    2. Press Select Study or Organization and select your study from the list. Or click the desired 'Project Name' from the Recently Accessed list.

    3. Click Reports and then Interactive Reports.

    1. Find and click the Device Shipment report under the Reports tab.

    2. The “Original U.S. Export Date” is the device’s Ship Date. Enter this date into the FSD.
